Compounded GLP-1
Compounded Tirzepatide Online, With a Licensed Provider
Through Forva Health, standard compounded tirzepatide plans cost $299 every 4 weeks, or $239 every 4 weeks on a 48-week prepaid plan ($2,868 upfront). Provider review, medication if prescribed, supplies, shipping and ongoing support are included, with no separate membership fee. Your price does not increase with your prescribed dose. Plans renew automatically until you cancel.
- ✓Licensed provider review
- ✓Medication if prescribed, supplies and shipping included
- ✓One price at every prescribed dose
- ✓Messages and dosing instructions in your patient portal
You pay at checkout. A licensed provider then reviews your intake and decides whether treatment is appropriate. Not everyone qualifies. If a provider decides treatment isn't appropriate, no medication is sent and you're refunded in full.
Compounded medications are not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness, or quality before they are marketed. A state-licensed 503A pharmacy prepares them for an individual patient when a licensed provider decides they are appropriate. Compounded Tirzepatide is not a generic version of, and is not the same as, Zepbound®.

How billing works
- ✓If a provider decides treatment isn't appropriate, no medication is sent and you're refunded in full.
- ✓You pay at checkout. Your first charge covers the plan length you choose. Prepaid plans ship 12 weeks of medication at a time.
- ✓After that, your plan renews automatically every 4 weeks at the same rate until you cancel, and the rate applies only while your subscription stays active. Your first renewal is charged 3 days early, counted from checkout: day 25 on the month-to-month plan, or 3 days before a prepaid plan's 12, 24 or 48 weeks end, so your refill ships on time.
- ✓To cancel, email support@forvahealth.com. Cancel before your renewal date to avoid the next charge. If a renewal has already been charged, cancel at least 72 hours before that refill is dispensed to stop the shipment. We confirm by email within 24 hours, and cancelling stops future billing.
- ✓Cancelling doesn't by itself entitle you to a refund, and medication that has already shipped can't be refunded. Cancelling before anything is dispensed refunds what you paid minus a $50 professional services fee. If a provider later stops your treatment or we can't fill your prescription, the 4-week cycles already dispensed are repriced at the $299 standard rate and any difference is refunded.

Questions before choosing a tirzepatide plan?
Compare the amount charged today, the length of the prepaid term and the ongoing renewal rate. Four weeks is 28 days, rather than a calendar month. A longer prepaid plan changes how much you pay upfront; your provider decides whether treatment and any dose changes are appropriate.
How it works
Step 1
Complete your intake
Answer questions about your health history and goals online, then choose a plan and pay at checkout.
Step 2
A licensed provider reviews it
Once you've finished the ID check and consent in your patient portal, an independent provider licensed in your state decides whether compounded tirzepatide is appropriate, typically within 24 hours. Not everyone qualifies, and some states require a short video visit.
Step 3
If prescribed, it ships to you
One of our partner pharmacies (AbsoluteRx, Strive Pharmacy or Boudreaux's New Drug Store, depending on your state) usually processes your order within a day and ships it with 2-day shipping. Dosing instructions and care-team messages arrive in your portal, and a short check-in there every 3 months keeps your prescription active.
Important safety information
GLP-1 medications can cause side effects, most commonly nausea, vomiting, diarrhea and constipation, and carry serious risks. Labeling for FDA-approved semaglutide and tirzepatide products includes a boxed warning about thyroid C-cell tumors. Your provider reviews these risks before prescribing.
Read the Zepbound® Prescribing Information, which includes the boxed warning, and the FDA's concerns with unapproved GLP-1 drugs used for weight loss.
Frequently asked questions
Is compounded tirzepatide FDA-approved?
No. Compounded tirzepatide is not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness, or quality before they are marketed. A state-licensed 503A pharmacy prepares it for an individual patient when a licensed provider decides it is appropriate.
Is compounded tirzepatide the same as Zepbound®?
No. Compounded tirzepatide is not Zepbound®, is not a generic version of Zepbound®, and is not the same product. Zepbound® is an FDA-approved tirzepatide medication made by Eli Lilly. Clinical trial results for Zepbound® describe Zepbound®. They do not apply to compounded tirzepatide. Providers in Forva's network can also prescribe Zepbound® when appropriate. If they do, you pay Lilly's self-pay price for the medication separately, and Forva's $99/month membership, which renews monthly until you cancel, covers provider review and ongoing care. The membership does not include medication, and it is refunded in full if a provider does not prescribe.
How much does compounded tirzepatide cost through Forva?
$299 every 4 weeks month to month. Prepaid plans cost less per 4 weeks: $269 on a 12-week plan ($807 charged at checkout), $259 on a 24-week plan ($1,554) and $239 on a 48-week plan ($2,868). Medication if prescribed, supplies, shipping and ongoing support are included, with no separate membership fee. Plans renew automatically every 4 weeks until you cancel. Compounded tirzepatide is not FDA-approved.
Is there a separate membership fee for compounded tirzepatide?
No. The compounded plan price includes provider review, medication if prescribed, supplies, shipping and ongoing support. The brand-name medication path has separate membership and medication charges. Compounded tirzepatide is not FDA-approved.
Does the price go up if my dose increases?
No. Your plan price stays the same at every dose your provider prescribes, and medication if prescribed, supplies, shipping and ongoing support are included.
Is this an introductory price?
No. The listed price isn't a first-month offer. Your plan renews at the same rate every 4 weeks, at every dose your provider prescribes, while your subscription stays active.
What if I'm not prescribed?
If a provider decides treatment isn't appropriate, no medication is sent and you're refunded in full.
What happens when my plan ends?
It renews automatically every 4 weeks at the same rate until you cancel, and the rate applies only while your subscription stays active. The first renewal is charged 3 days early, counted from checkout: day 25 on the month-to-month plan, or 3 days before a prepaid plan ends. The month-to-month plan renews at $299 every 4 weeks.
How do I cancel?
Email support@forvahealth.com. Cancel before your renewal date to avoid the next charge. If a renewal has already been charged, cancel at least 72 hours before that refill is dispensed to stop the shipment. We confirm by email within 24 hours, with no retention offer required, and cancelling stops future billing. Cancelling doesn't by itself entitle you to a refund, and medication that has already shipped can't be refunded; our refund policy explains the rest.
Do I need insurance?
No. Plans are self-pay and Forva doesn't bill insurance. You may be able to pay with an HSA or FSA card; whether a purchase qualifies depends on your plan, so check with your plan administrator.
How fast does compounded tirzepatide ship?
Once your intake, ID check and consent are complete, a provider typically reviews within 24 hours. If prescribed, the pharmacy usually processes your order within a day and ships it with 2-day shipping, at no extra charge. Prepaid plans ship 12 weeks of medication at a time. Your dosing instructions arrive in your patient portal, and a short check-in there every 3 months keeps your prescription active.
Which pharmacy fills my prescription?
One of our partner pharmacies: AbsoluteRx, Strive Pharmacy or Boudreaux's New Drug Store, all state-licensed 503A compounding pharmacies in the U.S. Which one fills yours depends on your state.
Is it a salt form of tirzepatide?
No. It's tirzepatide base, not a salt form such as tirzepatide sodium or tirzepatide acetate.
What follow-up care is included?
You can message the clinical team in your patient portal at any time. Your provider decides any dose changes, and a short check-in there every 3 months keeps your prescription active.
