Forva Health LLC operates two separate text messaging programs, described below.

Transactional messages relate to your account and care, including order and shipping notifications, prescription and clinician updates, appointment reminders, and responses to customer support inquiries.

Marketing messages include promotional content such as offers, discounts, and reminders to complete an unfinished intake. Consent to receive marketing messages is optional.

The two programs use separate opt-in methods, described below. Consenting to one does not enroll you in the other.

Consent is collected on our patient intake form at forvahealth.com/get-started/weight-loss. After completing the health questionnaire, you reach a contact step where you enter your first name, last name, email address, and mobile telephone number. The messaging disclosures appear directly beneath those fields.

Transactional messages. You opt in to transactional messages by providing your mobile telephone number on the intake form. The following disclosure is displayed at the point where your number is collected:

By providing your phone number, you agree to receive transactional texts from Forva Health about your order, prescription, and shipping. Msg & data rates may apply. Reply STOP to cancel.

Marketing messages. Marketing consent is collected separately, through a checkbox that is not pre-checked. Checking it is optional. You can complete your purchase without consenting to receive marketing messages, and leaving it unchecked does not affect your care, your order, or the price you pay.

The consent step appears on the intake form as shown below.

You represent that you are the subscriber to, or the customary user of, the mobile telephone number you provide, and that you are authorized to consent to receive messages at that number.

Message frequency varies and depends on your activity, your treatment plan, and the program or programs you have joined.

Message and data rates may apply. Forva Health LLC does not charge for text messages, but your mobile carrier's standard messaging and data rates will apply to each message sent or received. Contact your wireless provider for details about your plan.

You may cancel at any time by replying STOP to any message you receive from us. We will send a confirmation message, after which you will receive no further messages from that program.

Opting out of marketing messages does not automatically opt you out of transactional messages relating to your account and care. Opting out of text messages does not opt you out of email communications.

You must be 18 years or older to consent to receive text messages from Forva Health LLC and to use our Services. This messaging program is intended for individuals 18 years of age or older only. We do not knowingly send messages to, or collect mobile telephone numbers from, individuals under 18 years of age.

Message delivery is subject to effective transmission by your mobile carrier and is not guaranteed. Wireless carriers are not liable for delayed or undelivered messages. Not all mobile devices or handsets may support our messaging programs, and we may be unable to deliver messages to all carriers.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. We do not sell your mobile telephone number.

Your mobile telephone number is shared only with the service providers who deliver messages on our behalf, and those providers may use it solely for that purpose. Standard text messages are not sent over a secure or encrypted channel and may be visible to anyone with access to your device.

We may modify or discontinue any messaging program, in whole or in part, at any time and without notice. Continued participation after any change constitutes acceptance of the revised terms.
