Our Medical Team

Every treatment plan offered through Forva Health is reviewed and, where clinically appropriate, prescribed by a licensed healthcare provider. Providers evaluate each patient's health history and goals before recommending a course of treatment, and continue to be available for follow-up care throughout a patient's membership. Results vary by individual.

Required State Licensing

The following states require public disclosure of the supervising provider's name and license information:

Providers are licensed healthcare professionals responsible for all clinical decisions. Not all patients will qualify for treatment. Any medication is prescribed only following an individualized clinical evaluation and at the sole discretion of the treating provider.
