Member Journey
Step 1: Complete Your Health Assessment
Visit ForvaHealth.com and complete your online health assessment. This information helps a licensed healthcare provider understand your medical history, health goals, and determine whether treatment may be appropriate for you.
Step 2: Provider Review
After submitting your assessment, a licensed healthcare provider will review your information. Depending on your state's requirements, your consultation may take place in one of two ways:
Online Provider Review
In many states, providers can review your information and determine eligibility without a live appointment.
Video Consultation
Some states require a live video consultation with a licensed provider. If required, you'll be prompted to schedule your appointment after completing your assessment.
Your provider will determine whether you qualify for treatment and recommend the option that best aligns with your health goals.
Step 3: Begin Your Treatment Plan
Once approved, you'll select the treatment plan recommended by your provider and gain access to ongoing support from the Forva team.

Your medication will typically arrive within 5–10 business days after your order has been processed.
