Brand-name GLP-1
Foundayo™ Online, With a Licensed Provider
Foundayo™ (orforglipron) is an FDA-approved, once-daily GLP-1 pill made by Eli Lilly, taken with or without food. Through Forva, an independent provider licensed in your state reviews your intake and, if appropriate, sends your prescription through LillyDirect. Your Forva membership is $49 for your first month, then $99 a month, and you pay Lilly's self-pay price for the medication (from $149 a month) to the pharmacy that fills it.
- ✓Licensed provider review
- ✓Prescription sent through LillyDirect if appropriate
- ✓Lilly's self-pay price, with no markup from Forva
- ✓Dose changes, refills and care-team messages in your patient portal
Your $49 first month is charged at checkout, then the membership renews at $99 a month until you cancel. If your provider decides treatment isn't appropriate, your first month is refunded in full. The membership doesn't include the medication.
Foundayo carries serious risks, including a boxed warning. See important safety information.

What Foundayo costs through Forva
Forva charges your monthly membership: $49 for your first month, then $99 a month. The pharmacy that fills your prescription charges separately for the medication, at Lilly's self-pay price.
| Daily dose | Lilly self-pay price |
|---|---|
| 0.8 mg (starting dose) | $149 |
| Higher doses | Up to $299 |
Your provider increases the dose in steps, from 0.8 mg up to as much as 17.2 mg once a day, based on how you respond. At 14.5 mg and 17.2 mg, Lilly's $299 price requires each refill within 45 days of the previous delivery; a higher price applies outside that window.
Medication prices are set by Eli Lilly and can change, and Lilly sometimes lowers them for a limited time. The current price for each dose is on LillyDirect. This path is self-pay only: Forva doesn't bill insurance, and prescriptions sent through LillyDirect from Forva are self-pay.

How it works
Step 1
Complete your intake
Answer questions about your health history and goals online, choose Foundayo, and pay $49 for your first month of membership at checkout.
Step 2
A licensed provider reviews it
An independent provider licensed in your state decides whether Foundayo is appropriate. Not everyone qualifies, and some states require a short video visit.
Step 3
Buy the medication from Lilly's pharmacy partner
If prescribed, your prescription goes through LillyDirect to Gifthealth, its partner pharmacy, which texts or emails you to pay, usually within about 72 hours. The pharmacy ships the medication to you; dosing instructions and care-team messages are in your patient portal.
Important safety information
Foundayo carries a boxed warning about the risk of thyroid C-cell tumors. Don't take it if you or a family member has had medullary thyroid carcinoma (MTC), or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). The most common side effects are digestive, including nausea, constipation, diarrhea and vomiting. Your provider reviews these risks before prescribing.

Frequently asked questions
How much does Foundayo cost through Forva?
Forva charges the membership: $49 for your first month, then $99 a month. The pharmacy that fills your prescription charges separately for the medication, at Lilly's self-pay price: $149 a month at the 0.8 mg starting dose, and up to $299 a month at higher doses. Lilly sets these prices, sometimes lowers them for a limited time, and can change them. LillyDirect shows the current price for each dose.
What is Foundayo?
Foundayo (orforglipron) is an FDA-approved GLP-1 medication made by Eli Lilly. It's a pill you take once a day, with or without food. It's approved for adults with obesity, or with overweight and a weight-related health condition, alongside a reduced-calorie diet and more physical activity.
Is the medication included in the membership?
No. The membership covers your provider review, sending your prescription, dose changes and refills, and ongoing support. You pay for the medication separately, at Lilly's self-pay price.
Does Forva bill insurance?
No. This path is self-pay only. Forva doesn't bill insurance, and prescriptions sent through LillyDirect from Forva are self-pay.
What if I'm not prescribed?
If your provider decides treatment isn't appropriate, your $49 first month is refunded in full.
What if I'm prescribed but decide not to buy the medication?
Email support@forvahealth.com to cancel before your next monthly charge, and we'll close any open prescription. Because your provider visit has already taken place, your $49 first month isn't refunded.
Is there a refill rule?
At the 14.5 mg and 17.2 mg doses, Lilly's $299 self-pay price requires each refill within 45 days of the previous delivery. Outside that window a higher price applies.
Where does my Foundayo come from?
Your prescription goes through LillyDirect to Gifthealth, its partner pharmacy. Gifthealth texts or emails you, usually within about 72 hours, to confirm your details and pay Lilly's self-pay price, then ships the medication to you. The message may not mention Forva, so check your spam folder too. If you haven't heard anything after 72 hours, call Gifthealth at 833-707-6619.
How do I cancel the membership?
Email support@forvahealth.com at any time. Cancellation takes effect at the end of the current month, and you won't be charged again after that.
Is Foundayo the same as compounded semaglutide or tirzepatide?
No. Foundayo contains orforglipron, a different medication, and it's FDA-approved. Compounded semaglutide and tirzepatide are not FDA-approved and are separate options at Forva; your provider decides what's appropriate.
